Geriatric Medication Management
Geriatric Medication Management is a
9-month course covering key medication management issues for older
persons including medication reconciliation, barriers to medication
adherence and communication with physicians about medication-related
problems.
Geriatric Pain Management
Geriatric Pain Management is the second offering from the CHAMP
program. This 8-month course covers critical dimensions of caring for
the homecare patient with pain including pain screening and intensity
assessment, optimizing pain management and developing care plans for
older persons with pain.
