Recently published studies confirm that inappropriate medication use remains a serious problem for the elderly. A well established tool to identify potentially inappropriate medications is the set of Beer’s Criteria for Potentially Inappropriate Medication Use in Older Adults (Criteria). The Criteria identify medications noted to have potential risks that outweigh potential benefits by an expert consensus panel. Designed for population-based screening, the Beer’s criteria can be used to determine if there is evidence of inappropriate drug prescribing. Some CHAMP participants have used the Beer’s list for screening to identify patients who may be at risk for medication-related problems. The Hartford Foundation Institute for Geriatric Nursing’s “Try This” overview succinctly reviews the tool’s validity and reliability, and strengths and limitations.

"Try This" issue that focuses on disease and condition-specific medication concerns.  It includes a list of 20 diseases/conditions and the associated medications that may increase the risk of adverse reactions. Published by the Hartford Institute for Geriatric Nursing.

In 2005, under the direction of the Centers for Medicare & Medicaid Services, the Quality Improvement Organization (QIO) Program began work on reducing avoidable hospitalizations in home health. This work culminated with the 2007 Home Health Quality Improvement (HHQI) National Campaign in which over 5600 home health agencies pledged to work to reduce avoidable hospitalizations. This 12 month campaign featured a new Best Practice Intervention Package (BPIP) for each month, focusing on topics that were under the control of the agencies to help reduce hospitalizations. These topics included hospitalization risk assessments, emergency care plans, medication management, phone monitoring and front loading visits, teletriage, telemonitoring, immunizations, physician relationships, fall prevention, patient self-management, disease management, and patient care coordination. Each package contains tools and strategies for reducing hospitalizations. All packages contain leadership tracks and individual discipline-specific tracks for nursing, therapy, social services, and home health aides.
