Supervisors face challenging work situations every day that require independent thinking and decision making along with the ability to juggle competing priorities. These often overwhelming positions would be less stressful if workers were consistently reliable and possessed the ability to resolve problems effectively and independently. The PHI Coaching Approach to Supervision: Introductory Skills for Supervisors in Home and Residential Care introduces supervisors of direct-care workers to the skills they need to effectively help workers solve problems and improve work performance. This curriculum is designed to provide the necessary tools to build positive relationships within the context of challenging work settings. The 7 modules are intended to be taught over two days, and train supervisors in four key skills: Active Listening, Self-Management, Self-Awareness, and Presenting the Problem.

Continuity Assessment Record & Evaluation (CARE) Instrument

This internet-based instrument developed by CMS provides an electronic means for a variety of health care providers to rapidly and accurately communicate critical patient information during transitions.

Timely communication of patient information between discharging and receiving care providers is essential to supporting safe care transitions. The Centers for Medicare & Medicaid Services (CMS) has developed, and is currently piloting, the internet-based Continuity Assessment Record & Evaluation (CARE) Instrument, which provides an electronic means to rapidly and accurately communicate critical patient information during transitions. CARE captures key information such as current medications, discharge care needs and functional ability to help care providers to improve care coordination for their patients.
