Pain that is not assessed goes untreated and pain not reassessed following an intervention may be undertreated. Health care providers need to make the assessment of a patient’s pain a top priority. The patient is the only one that can describe their experience of pain, therefore, self-report tools are considered to be the gold standard for assessing pain. Describing pain using a succinct, yet standardized pain scale helps patients to verbalize their pain care needs. No one pain scale is the right pain scale for every patient. The right pain scale is the one understood by the individual patient. Once identified, this scale should be documented as the scale for that patient and used consistently. Health care providers can then act on the expressed pain needs of the patient.

Pain Management Constipation Algorithm

A one page decision-tree to help clinicians effectively manage constipation in patients taking opioid medications. It is also useful for agencies to standarize bowel care in such patients.

Initial and on-going assessment of constipation is an essential component of pain management. Because constipation is the most common opioid side effect, it should be aggressively prevented and managed with a proactive bowel management program. The Pain Management Constipation Algorithm distills established components of bowel management into a convenient tool that agencies can use for staff education and to standardize bowel care.

Pain Management Zone Tool

This tool assists patients/caregivers in managing pain using red, yellow and green zones. It also helps patients determine when to call the physician or home care nurse and/or go to the emergency department.

Patient Health Questionnaire (PHQ-2)

A validated two-item screener for depression that has also been incorporated into OASIS-C tool as item M1730.  It asks about the frequency of depressed mood and loss of interest/pleasure in daily activities. Patients who screen positive can be further evaluated with the PHQ-9.

Patient Health Questionnaire (PHQ-9)

A multi-purpose tool to assist clinicians with identifying, diagnosing, monitoring, and measuring the severity of depression. This simple questionnaire should be administered if the PHQ-2 depression screener is positive.

Peer mentoring programs can help employers reduce turnover among their direct-care staff. The programs instill newly hired workers with the confidence and skills they need to become successful, as well as provide veteran direct-care workers with a chance for career advancement. Peer Mentoring: A Workshop Series for Direct-Care Workers in Home and Residential Care is an eight module, 30-hour workshop series. Its content can be used to teach mentors-in-training critical relational skills, including leadership skills, problem solving, and the ability to communicate effectively. This adult learner-centered training, involves trainees in numerous skill-building activities -- role playing, presentations, brainstorming, discussions -- in both large and small groups. This participatory approach to training is critical to develop effective peer mentors.
