Just saw this tool for the first time. Good starting point; I will use the AACE guidelines rather than the ADA because they are tighter.

VNSNY now uses the 2009 ADA guidelines for fasting blood glucose. In the Green Zone it should be stated: Fasting blood glucose = 70 - 130 mg/dl

The diary was developed as part of a multi-faceted intervention of the Healthy Living as You Age study, lead by Dr. Alison Moore, a professor of medicine in the division of geriatrics at the David Geffen School of Medicine at UCLA.

Eating and Feeding Issues in Older Adults with Dementia: Part I: Assessment (Try This, Issue D11.1)

"Try This" issue that describes the Edinburgh Feeding Evaluation in Dementia Questionnaire (EdFED-Q), a tool to identify eating and feeding difficulties for persons with late-stage dementia.  Originally developed for hospitalized patients, the EdFED-Q may also be useful for home care patients.
