We use Honeywell HomMed telemonitors. We have reduced out hospitalization rate from 30 % to 25%. We also did education with our evening on-call nurses to be sure they would make a visit and assess the clients rather than just say "Call your MD" or "go to the ER". (unless of course it was a true emergency)
