Follow the instructions below to register for the CHAMP Helping Patients Adhere course.

Tuition and Fees
$35 per participant (For members of state home care associations in Arkansas/Colorado/Florida/Georgia/Illinois/Iowa/Maine/Massachusetts/Minnesota/Montana/New Hampshire/New Jersey/New Mexico/New York/Oregon/Pennsylvania/Rhode Island/Washington and California agency-based participants).

$49 per participant (all others).

Cancellation Policy
Cancellations will be refunded at 100% of tuition and fees paid, less a $10 processing fee prior to starting the course. No cancellations will be accepted once you have begun the course. To cancel your registration please contact Thomas Bow via email at thomas.bow@vnsny.org.

Deadline for Completion
While there is no official completion deadline, due to space limitations access to the online course cannot be guaranteed beyond 3 months from your course registration date.

Continuing Education Hours
All Registered Nurses will receive 3.6* continuing education contact hours upon completion of the course.  Partial credit will not be given for partial completion of the course.

No continuing education hours will be given for this course beyond December 31, 2014.

*California licensees will receive 4.3 CE contact hours, due to differences in how the state of California calculates these hours.
