The CHAMP Program launched its new web site June 30, 2009, with an all new look and design. The site has tools, evidence-based best practices, and an online Community that features discussion forums and Ask the Expert blogs.

CHAMP Releases Framework “Best Practices” for Home Care
In late Spring, CHAMP released a series of Evidence Briefs from the “National Framework for Geriatric Home Care Excellence”. The first-of-their-kind for home care, the Briefs outline key best practices and recommendations for key practices in home care for older persons.

Measurable Results from CHAMP’s Medication Management Course
In early January, CHAMP released the results achieved by the first two classes of participants in its Geriatric Medication Management course. Significant improvement was achieved in important care processes and patient outcomes for older persons.
