CHAMP Program Geriatric Medication Management ImageMost older home care patients are taking more than 5 medications. Medication-related problems contribute to poor patient outcomes and are a leading cause of avoidable rehospitalizations.

CHAMP’s Medication Management course is designed to give frontline home care managers the skills and tools they need to help their clinical team improve medication management for older home care patients. The course includes best practices in medication management for home care patients, including:

  • Medication Reconciliation
  • Identifying drug/drug interactions and medication-related complications
  • Improving management of oral medications
  • Communicating with physicians about medication issues

Four online units, coupled with practical exercises and downloadable tools, help home care managers and their teams significantly improve care. Click here for a summary of the course objectives by unit.
