CHAMP Program Geriatric Pain Management Rating Scale

Pain Hurts!

Pain management for older patients is a key quality of life issue, but only 60% of patients are discharged from home care with less pain than when they were admitted.

CHAMP’s Geriatric Pain Management course is designed to give frontline home care managers the skills and tools they need to help their clinical team improve pain management for older home care patients. The course includes best practices in caring for the home care patient with pain, including:

  • Pain screening and intensity assessment
  • Pharmacologic and non-pharmacologic pain relief
  • Developing care plans for older persons with pain
  • Communicating with physicians about pain issues

Five online units, coupled with practical exercises and downloadable tools, help home care managers and their teams significantly improve care. Click here for a summary of the course objectives by unit.
