Visit our newly released CHAMP Best Practices Continuing Education Corner.  This page contains short learning activities based on select Framework Evidence Briefs in the areas of Care Coordination, Management & Transitions, Medication Management and Palliative Care & Advanced Illness Management.  Completion of these earns registered nurses 1 continuing education contact hour and at $10 each, they are very affordable!  Click here to learn more.

 

Registration for CHAMP's Geriatric Medication Management and Geriatric Pain Management courses beginning April 26, 2010 is now open. 

The newly redesigned CHAMP Geriatric Medication Management and Pain Management courses are proven to improve patient outcomes and quality of care, and both are:

Designed for Success

Evidence-based, and developed by experts in home care, CHAMP’s courses are self-paced. Participants schedule time for CHAMP assignments and activities at their convenience. Web-based e-learning units blend clinical information with practical tools and strategies for improvement. Participants in CHAMP courses learn by doing, which increases effectiveness and staff morale.
